What is the height (from the bottom) of each of the 4 different horizontal holes?

The heights of the 4 horizontal holes (from the bottom) are: 3.9 inches, 5.9 inches, 11.8 inches, 13.8 inches.

How can you get metal bars to extend the shelf?

The smaller diameter bars slide into the larger diameter bars. They can be shortened or lengthened by sliding them in/out to get the exact length desired.

Is this length only adjustable to 20.7″ or 32.3″, or can it be adjusted to 28″ also?

The rods are telescoping, so it is adjustable.The maximum length is 32.3 “. The shortest is 20.7 ” . Ours is set at 26″.
